If my child had an ImPACT Concussion baseline test in 2017-18, do they need another in 2018-19?
    No. Any student-athlete who took an ImPACT Concussion baseline test at any time during the 2017-18 academic year, as administered by District personnel, does not need to take another ImPACT Baseline concussion test in 2018-19.  These tests are to be administered every other year. Only those students who didn't have a baseline test in 2017-18, are new to the District, new to Athletic Participation and/or entering the 7th grade need an ImPACT test in 2018-19.
    What forms do I need to print off and turn in to the Athletic Department?
    PIAA CIPPE Sections 1, 2, 5, & 6 along with Drug Test Receipt. All other forms may be completed electronically online via the Parent Portal system.  The PIAA CIPPE Sections 1, 2, 5, & 6 must be turned into the Athletic Office.  The drug test consent form is turned in at the time of testing along with the $30 fee - checks made payable to S.V.S.D. Keep drug test receipt (the carbon copy paperwork from actual testing agency) and turn in along with PIAA CIPPE FORM Sections 1, 2, 5, & 6 to the Athletic Office.

    What forms are required for multi-sport athletes?
    PIAA CIPPE Section 7. Once a student-athlete completes the initial paperwork for their first sport (CIPPE Sections 1, 2, 5, & 6 along with drug test & concussion test requirements) the only required paperwork for their subsequent sports is PIAA CIPPE Section 7.

    How do I confirm receipt of the Student-Athlete Manual?
    Via the Parent Portal.  The Student-Athlete Manual cover page is to be signed off on electronically, by both student and parent/guardian, via the Parent Portal.  Please, do NOT print off this form.
    What do I do if my child is not enrolled in a District school and I don't have access to Parent Portal?
    Contact the Athletic Office at 724/452-6040, x1752. Any student who lives in the District but does not attend a District school (i.e., homeschool) and wishes to participate in District athletics must complete all necessary paperwork.  Contact the Athletic Office and information will be provided.
    Who should complete the student-athlete registration process?
    7th - 12th graders. Any student - in grades 7th - 12th - interested in participating in District and/or Club Sports must complete the Student-Athlete registration process.
    What if I can't find my Parent Portal access code or information card?
    Contact the School District Main Office.  If you cannot find your Parent Portal access code or information card, please contact the District main office at 724/452-6040, x 1642.  The Athletic Office cannot provide this information.  
    UPDATED: When must the physical be completed?
    Forms must be dated after June 1st, 2018.  As of May 1, 201,6 the PIAA has notified all member schools that the requirement for the PIAA CIPPE Form Section 6 is that an Authorized Medical Examiner must sign off on this section on or after June 1, 2018 - regardless of the date of the actual physical examination.  It is possible for a physical exam to be administered prior to June 1, 2018, but the PIAA CIPPE Form Section 6 must be dated as of June 1, 2018.
